Responsibilities
As a Registered Nurse in Home Health Care, you will be the central point of contact for all disciplines involved in patient care and will oversee the coordination of care visits. Your responsibilities will include consulting with physicians regarding patient needs, performing accurate OASIS collection, keeping medication profiles current, ensuring timely reporting of lab values to physicians, and actively participating in weekly case conferences and monthly case manager meetings.
