Summary
JOB SUMMARY :
- Under direction of the Director of Ambulatory and Physician Practice Outcomes, the Ambulatory Quality Outcomes
- Coordinator collects, analyzes, interprets and facilitates quality improvement and systems transformation activities among LifeBridge Physician Network ambulatory practices.
- Participates in quality review, data collection and analysis, and improvement facilitation to ensure optimal patient care and practice adherence with regulatory standards.
- Works with practice providers, managers and staff to educate and ensure oversight of relevant quality measures, identifies areas of concern, and participates in improvement efforts. Facilitates attainment of LifeBridge Physician Network (LBPN) quality priorities and elements of the NCQA Patient-Centered Medical Home model for primary care practices.
