We are currently seeking a compassionate and dedicated Social Worker to join our team at our skilled nursing facility. This is a full-time opportunity for someone who is passionate about resident advocacy, care coordination, and supporting residents and families through every stage of their stay.

Key Responsibilities :

Advocate for residents’ psychosocial needs and overall well-being

Complete psychosocial assessments and care plans

Assist with admissions, discharges, and transitions of care

Coordinate with interdisciplinary teams, residents, families, and outside agencies

Provide counseling, emotional support, and crisis intervention as needed

Ensure compliance with state and federal regulations

Support resident rights and promote quality of life

Qualifications :

Bachelor’s or Master’s degree in Social Work (BSW or MSW required)

Experience in a skilled nursing facility or healthcare setting preferred

Knowledge of discharge planning and community resources

Strong communication, organizational, and documentation skills

Compassionate, professional, and resident-focused approach
