About the Role:
The Housing Support Specialist is essential in connecting individuals and families facing housing instability with vital housing resources and healthcare support services. You will play a key role in assessing client needs, identifying suitable housing options, and facilitating access to community resources, ultimately leading to long-term housing stability and better health outcomes. Collaboration with healthcare providers, social service agencies, and landlords is paramount to ensure seamless transitions into secure and affordable housing. Your advocacy and guidance will empower clients by reducing barriers to housing and helping them maintain their living arrangements, contributing positively to their overall well-being.
Minimum Qualifications:
- Bachelor's degree in social work, psychology, biology, public health, nursing, community health, or a related field, or equivalent experience.
- A minimum of 5 years of experience working with individuals facing complex health and social issues.
- Proven knowledge and experience with community agencies and programs.
- Familiarity with Medi-Cal eligibility guidelines and the application/renewal process.
- Strong problem-solving skills and exceptional customer service abilities.
- Comprehensive understanding of the DHCS housing and tenancy program, including services and requirements.
- Proficient in, or willing to learn, HMIS and other data entry systems.
- California residency is required, and candidates must maintain residency while employed.
- Valid California Driver's License and a personal vehicle are necessary.
- Current auto insurance, meeting California's minimum coverage requirements, is needed.
- BCLS CPR Certification is required.
Preferred Qualifications:
- Certification in case management or housing navigation.
- Experience in healthcare settings or with interdisciplinary care teams.
- Familiarity with electronic health records (EHR) and case management software.
- Bilingual abilities or proficiency in languages relevant to the community served.
- Training in trauma-informed care and cultural competency.
Responsibilities:
- Conduct thorough assessments to identify clients' housing needs and barriers, collaborating to develop Individualized Service Plans (ISPs) tailored to housing, income maintenance, and personal goals related to medical, mental health, or social support.
- Offer referrals and support to resources that aid participants in achieving their ISP goals.
- Perform program-specific assessments.
- Evaluate strengths and challenges while conducting a 90-day review post-initial intake to address short- and long-term goals.
- Develop personalized housing plans in collaboration with clients and multidisciplinary teams.
- Stay informed about available housing resources, subsidies, and community programs.
- Advocate for clients with landlords, housing authorities, and service providers to secure housing placements, focusing on landlord engagement and negotiation.
- Provide ongoing support and follow-up to ensure housing stability and address any emerging challenges.
- Coordinate with healthcare providers to integrate housing solutions with clients' health and social care plans.
- Maintain accurate documentation of client interactions, housing plans, and outcomes, adhering to organizational policies.
- Participate in community outreach and educational efforts to raise awareness about housing resources and services.
