**Job Description**


Elmora Family Practice is currently seeking for a professional Bilingual [Spanish & English] Nurse Practitioner who can deliver patient-centered Primary Medical Care to a wide variety of patients in a community setting. The Provider will be responsible in caring for patients, maintaining accurate and current patient records, and administering follow-up to patients as required. The successful candidate will work alongside our current APN and MD. As a team we will continue to deliver premium care to each and every patient in our community.


Advanced Nurse Practitioner responsibilities:

• Practice in accordance with the state’s laws, rules and regulations.
• Practice as a member of an interdisciplinary health care team for all patients (13 and older).
• Provide primary health care through direct care, consultation, collaboration, coordination and/or referral.
• Assess and treat patients for a variety of chronic and acute conditions
• Review any and all results from ordered testing
• Update patient records and check records for accuracy at each patient appointment
• Perform initial patient screenings and patient history and physical examinations
• Appointments scheduled 5 days a week; Weekends and Major Holidays Off

Advanced Nurse Practitioner Qualifications:

• Bilingual [Spanish & English] (Mandatory)
• Valid Nurse Practitioner / Physician Assistant license
• Must have exceptional bedside manner and confidence
• 4+ years clinical experience
• Must maintain current license and certification during duration of employment
• Ability to work within a team and take direction
• Strong written and interpersonal communication skills
• Experience using EHR/EMR
