Job Description
Implements the treatment plan in collaboration with the treatment team. Provides a variety of
peer support, advocacy, and other recovery services to clients.
Job Responsibility
1.Collaborates with the treatment team to facilitate implementation of the treatment plan.
2.Provides warm hand-off to participants prior to discharge from hospital to outpatient services.
3.Provides a variety of peer support, advocacy, and other recovery services to clients and caregivers.
4.Develops a relationship of trust with each participant, functioning as a role model, peer support, mentor, advocate, and ally.
5.Communicates hope and encouragement to assist participants in developing readiness to discharge from the hospital and return to their community.
6.Provides case management service to identified clients while maintaining the wellbeing of clients and integrity of the program.
7.Offers assistance with independent living skills, and community adjustment, helping the individual make a positive connection to outpatient
behavioral health services and other community resources.

Job Qualification
- High School Diploma or equivalent required.
- Current Certified Recovery Peer Advocate or Certified Recovery Peer Advocate - Provisional certification preferred.
- 1-3 years of relevant experience, required.
