( LVN Preferred – Must Have Hospice Experience )

We are seeking a compassionate and professional Hospice Outreach Counselor to join our team in San Diego, CA . This role is ideal for an LVN (preferred), Chaplain, or Social Worker with hospice experience , who is passionate about supporting patients and families during a critical time in their lives.

In this role, you will serve as a key liaison between our organization, patients, families, and community partners—educating on services, securing consents, and ensuring emotional and informational support throughout the care journey.

Responsibilities

• Establishes and maintains positive working relationships with current and potential referral and payer sources
• Builds and monitors community, customer, and patient perceptions
• Introduces and completes Explanation of Benefits with patients and / or patient families
• Ensures consents are completed in accordance with appropriate guidelines and regulatory government agencies
• Maintains comprehensive working knowledge of service markets including government agencies, major payer groups, key referral sources and competitor market positioning
• Maintains comprehensive working knowledge in the field of marketing and sharing information with appropriate organization personnel
• Participates in the development of the individualized plan of care
• Counsels patient and family / caregivers and / or connecting to appropriate team resources as needed for stress and other coping mechanisms
• Occasional on-call responsibility as appropriate for discipline
• Ensures compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

Salary Range

USD $35.00 - $40.00 / Hour
