Job Description

The Disclosure Management Coordinator is responsible for organizing and overseeing the record-keeping activities pertaining to retrieving and disclosing medical records.

ESSENTIAL / CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES

1.                   Receive and process all medical record requests (active and closed) within the Agency in accordance with agency policies and procedures, and ensure all medical records are disclosed in accordance with HIPAA guidelines

2.                   Document all requests for records and status of requests in the Health Information Exchange (HIE) portal

3.                   Follow-up with staff and external parties upon receipt of deficient requests for records and provide guidance on correcting the request

4.                   Document the receipt and disclosure of all record requests within the electronic medical record (i.e., scanning requests in the EMR, documenting disclosures in the Disclosure Management and Individual Notes in the EMR)
