Job Summary

As a Weekend Registered Nurse (RN) with Buena Vista Home Health , you will plan, organize, and direct home care services. You'll work closely with patients, families, and the interdisciplinary team to deliver high-quality care and education in the home setting.

Responsibilities

  • Develop and initiate individualized plans of care
  • Provide nursing services ordered by the physician
  • Educate patients and caregivers on health management
  • Prepare clinical notes and maintain accurate documentation
  • Communicate with physicians and healthcare team
  • Identify discharge planning needs and implement prior to discharge

    • Requirements

  • Active California RN License
  • Minimum 2 years of nursing experience (home health or public health preferred)
  • Current CPR certification
  • Licensed driver with insured, reliable vehicle
  • Strong observation, problem-solving, and communication skills
  • Ability to lift and transfer patients and perform physical tasks
