Home Health Physical Therapist

A home-health hospice facility based out of Modesto, CA is seeking an experienced Physical Therapist to join their team!

Physical Therapist Salary: $125,000 - $145,000/annually

Physical Therapist Opportunity:

• Full-time, direct hire
• Schedule: Variable; seeing between 2-8 patients a day
• Setting: Home Health
• Assess and develop care plans : Conduct comprehensive evaluations of patients' physical abilities and design individualized treatment plans in collaboration with the interdisciplinary care team
• Implement interventions : Provide therapeutic exercises and mobility training to maintain or improve function, comfort, and quality of life for patients with terminal illnesses
• Educate and support caregivers : Train family members or caregivers on proper body mechanics, safe transfer techniques, and ways to assist with daily activities to ensure patient comfort and safety at home

Physical Therapist Qualifications:

• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• A license to practice Physical Therapy in California
• At least 1 year of Physical therapy experience required
• Experience in home health setting is a plus
