About the Role:

The Home Health Aide plays a critical role in providing compassionate and personalized care to patients within their homes, ensuring their comfort, safety, and well-being. This position involves assisting individuals with daily living activities, monitoring their health status, and supporting their emotional and physical needs in a variety of settings including hospice, assisted living, and long-term care environments. The aide collaborates closely with healthcare professionals and family members to implement care plans tailored to each patient's unique requirements. By maintaining a clean and sanitized environment, the aide helps prevent infections and promotes overall health. Ultimately, the Home Health Aide contributes significantly to enhancing the quality of life for patients who require ongoing support outside of traditional clinical settings.

Minimum Qualifications:

• High school diploma or equivalent.
• Certification as a Home Health Aide (HHA) or equivalent state certification.
• Experience working in home health, hospice, or long-term care settings.
• Basic knowledge of patient care techniques and safety protocols.
• Ability to pass background checks and health screenings as required.

Preferred Qualifications:

• Previous experience in assisted living or hospice care environments.
• CPR and First Aid certification.
• Additional training in sanitization and infection control procedures.
• Strong communication skills and ability to work independently.
• Familiarity with electronic health record (EHR) systems.

Responsibilities:

• Provide direct patient care including assistance with bathing, dressing, grooming, and mobility.
• Monitor and report changes in patients’ physical or emotional condition to healthcare supervisors.
• Assist with medication reminders and ensure adherence to prescribed care plans.
• Maintain a clean, safe, and sanitary environment in patients’ homes to prevent infection and promote health.
• Support patients in hospice and long-term care settings with dignity and respect, addressing their comfort and emotional needs.
• Communicate effectively with patients, families, and healthcare team members to coordinate care and address concerns.
• Prioritize tasks efficiently to meet the varying needs of multiple patients throughout the day.
