Responsibilities
The Nurse Practitioner will conduct annual risk assessments in patients' homes or via telehealth, operating within the scope of practice and under the guidance of the Collaborative Physician.
Essential Duties and Responsibilities
Conduct comprehensive in-home health risk assessments to identify all active and chronic conditions and assess physical, mental, and social needs during each visit.Take patient histories, perform examinations, and establish diagnoses.Provide detailed documentation of patient visits in accordance with NCQA standards.Record patient vital signs as required.Refer cases to case management and communicate with primary care providers as necessary.Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnoses.Perform all clinical duties while adhering to OSHA Universal Precautions.Maintain patient confidentiality.Attend necessary meetings and in-services and participate in committees as required.Engage in professional development activities to maintain licenses and affiliations.In this role, you will interact with :

Required Knowledge, Skills, and Experience
Active, unrestricted nurse practitioner license.Board certification from ANCC, AANP, or NCCPA.Current CPR Certification.Enrollment in Medicare / Medicaid.Valid driver's license and a clean driving record.Excellent EHR skills.Preferred Knowledge, Skills, and Experience
Experience in geriatric care.Strong teamwork skills and the ability to maintain effective relationships with patients, medical staff, and the public.Conditions of this role to be aware of :
Ability to adapt to varying weather conditions and the different environments of patients' homes.Ability to perform a full range of body motions, including the handling / lifting of patients, manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, as well as standing, bending, walking, and stair climbing.Regular lifting / carrying of items weighing up to 50 pounds.Ability to travel by automobile or van up to 150 miles daily in both urban and rural settings.Posted Min Pay Rate
