Position Overview :
Will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy. Focus on management of complex disease states in specialty clinics to help patients reach optimal therapeutic goals. Simplify patient medication regimens, minimize the risk of harmful medication interactions, optimize medication selection / dosing tailored to the patient and educate patients to enhance adherence and clinical outcomes. Responsible for policy development, continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship. Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies. Monitors clinical outcomes, communicates with medical providers, performs electronic documentation and refers patients to other health care providers. Pharmacy residency strongly preferred.

Pay Range is $97.37 to $107.10 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
