Job Status : Full-Time, Monday-Friday day shifts

Position Summary :

The Social Worker delivers varied social work services to Hospice patients and their families. To provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services and community education, outreach and referral. The Hospice social worker is an integral part of the Hospice IDG.

Required Experience for Social Worker :

Must have MSW degree from a school of social work accredited by the Council on Social Work Education one year of experience in a health care setting.

While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and / or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Must possess a valid driver's license, auto liability insurance, and reliable transportation. Travel to different locations may be required.
