Summary

The Supervisory Advanced Practice Provider oversees Physician Assistants and Nurse Practitioners at the DHS IHSC facility, training, and schedules. Provides direct detainee care, performs evaluations, and prescribes medications. Requires licensure, certification, and 6+ years clinical experience. The Supervisory APP reports directly to the facility Clinical Director.

This position is only open to USPHS officers and Call to Active Duty (CAD) candidates.

DUTIES AND RESPONSIBILITIES:

  1. Serves as the direct supervisor and rater to federal Advanced Practice Providers (APP) to include both Physician Assistants and/or Nurse Practitioner at IHSC sites.
  2. Provides leadership and oversight of APP onboarding and training.
  3. Participates in national APP training initiatives as required.
  4. Assist as required in the credentialing and privileging processes.
  5. Oversees the APP peer review processes.
  6. Augments clinical leadership in collaboration with the clinical director.
  7. Coordinates APP work schedule at the IHSC sites.
  8. Coordinates regular on-site APP meetings and in-service training as required.
  9. Serves as Acting Regional APP or Acting Chief APP as required.
  10. Provides direct care to detainees via scheduled appointments, emergency or urgent care visits.
  11. Works within and performs functions as outlined in the IHSC Advanced Practice Provider (APP) Collaborative Practice and Prescriptive Authority Agreement. (CP/PAA).
  12. Consults with the physician/Clinical Director for complex medical and/or psychiatric patients and as required by IHSC policy.
  13. Provides infirmary care including admission, discharge, and daily rounds for detainees that require higher levels of medical care or monitoring.
  14. Performs comprehensive history and physical evaluations, behavioral health care, and minor outpatient surgical procedures for detainees as required by IHSC policy, or as required for detainee medical complaint.
  15. Applies knowledge of drugs and therapeutics when prescribing medication.
  16. Coordinates health care needs of detainees including referrals to off-site consultants.
  17. Interprets diagnostic and laboratory results.
  18. Interprets Tuberculin skin tests (TSTs) and diagnostic studies, manages Direct Observation Therapy (DOT) for treatment of active or latent tuberculosis.
  19. Takes appropriate infection control actions in accordance with IHSC policy and guidance.
  20. Adheres to universal precautions and properly disposes of all sharps.
  21. May serve as medical escort of detainees as needed, for domestic/overseas travel.
  22. Utilizes and documents interpretation and/or translation and/or assisted communication services as needed to ensure ability to communicate with detainees or others who are Limited English Proficient (LEP) or have a communication disability or impediment.
  23. Provides relevant patient health education to detainees verbally and/or with handouts.
  24. Documents detainee care visits in the required health record using required format, in compliance with IHSC policy and guidance.
  25. Maintains patient confidentiality and confidentiality of medical records.
  26. Maintains basic readiness for duty.
  27. Maintains current CPR/BLS certification.
  28. Maintains clinical professional skills via continuing education opportunities.
  29. Completes all training as required/assigned.
  30. Maintains flexibility and adaptability to sudden changes in schedules and work requirements.
  31. Performs other duties and responsibilities as assigned.

  • Service Remaining Requirement Agreement: Upon appointment to this position, officers will be required to serve an 18 to 24-month commitment to IHSC.
  • ASSIGNMENT PAY AGREEMENT: You must not have a current Assignment Pay Agreement. However, if you have a current Assignment Pay Agreement, it must be fulfilled within 90 days of your application submission.
  • Applicants must be a current USPHS Commissioned Corps Officer or USPHS Call to Active Duty (CAD) candidate.

    Civilians are not eligible for this position.
  • PHS officers must participate in IHSC TDY On-Call Schedule (ITOS.) ITOS is either an entire month or two 16-day periods a year where, depending on the needs of the agency, the officer is required to be available to serve.

PHYSICAL DEMANDS:
  1. Requires physical exertion such as lifting objects greater than 30 pounds.
  2. Required to perform CPR/emergency care standing or kneeling.
  3. Must have the ability to assist sick, injured or aging detainees or staff exiting the building (may require lifting, dragging, wheeling or carrying someone who weighs significantly more than self).
  4. The work may necessitate some physical characteristics that will be required when responding to an emergency situation, such as running a short distance, prolonged standing and working in an austere environment.
Education

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
  1. Physician Assistant - Requires unrestricted Physician Assistant state license, national board certification through the National Commission on Certification of Physician Assistants (NCCPA).
    • Current Licensure as a Physician Assistant - Certified by a State, the District of Columbia, the Commonwealth of Puerto Rico, or territory of the United States.
    • Graduate from a Physician Assistant Program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA). Certified as a PA-C by the National Commission on Certification of Physician Assistants (NCCPA).
  2. Nurse Practitioner - Requires unrestricted Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner state license, national board certification through the American Academy of Nurse Practitioners (AANP) or the American Nurse Credentialing Center (ANCC).

    • Current licensure as a Nurse Practitioner - Certified by a State, the District of Columbia, the Commonwealth of Puerto Rico, or territory of the United States.
    • Certified advance practice nurse practitioner degree from a graduate school of training program approved for this qualifying degree.
    • Knowledge of the Nurse Practice Act for the licensing state and/or the state in which one practices nursing.
  3. Requires more than six years of full-time clinical experience as a PA or NP or approval of the IHSC Chief Advanced Practice Provider.
  4. Team lead or supervisory experience of more than two years.
  5. Fulfills the obligation(s) of their licensing or certifying body to maintain currency.
  6. Maintains unrestricted license with state of licensure and the Drug Enforcement Agency (DEA) to prescribe controlled medications.
Preferred Knowledge, Skills, and Abilities:
  1. Experience in IHSC health system.
  2. Correctional Healthcare experience.
  3. Certification as Correctional Healthcare Professional (CCHP)
  4. Knowledge and/or experience with IHSC policies, NCCHC and PBNDS requirements.
  5. Knowledge of public health preparedness, infection control and TB management
  6. ACLS certification.
  7. ATLS Certification.
Additional information

This position is located within the Department of Homeland Security (DHS), Immigration and Customs Enforcement (ICE), Enforcement and Removal Operations (ERO), ICE Health Service Corps (IHSC), Office of Deputy Assistant Director of Clinical Services. The incumbent will report directly to the site Clinical Director. This is a supervisory position. The Regional Advanced Practice Provider serves as the incumbent's reviewing official.

IHSC has a multi-sector, multidisciplinary workforce of more than 1600 employees, including U.S. Public Health Service (PHS) commissioned officers, federal civil servants, and contract staff. The IHSC provides on-site direct patient care to ICE detainees at 20 detention facilities throughout the country and manages the provision of off-site medical care for detainees housed in approximately 240 additional Intergovernmental Service Agreement (IGSA) facilities. The ICE detainee population is approximately 34,000 detainees on a daily basis, with an average length of stay of approximately 30 days, and over 400,000 detainees annually. The IHSC also provides medical support during ICE enforcement operations in the air, on the ground and at sea.
