Medical Social Services / LMSW PRN- Nashville

Medical Social Services position :

All Heart Home Care is seeking a dedicated PRN Social Worker to lead the development of a new business line specializing in client advocacy, guardianship, and essential social services. In this role, you will initially support home health clients, addressing their social and emotional needs, coordinating resources, and advocating for their well-being. As the new service line grows, you'll transition into a full-time role focused on client advocacy, guardianship, and social services. Ideal Candidates are proactive, compassionate, and eager to build meaningful connections while expanding services to support our clients' diverse needs. We are currently interviewing for a PRN position out of our Nashville office.

QUALIFICATIONS :

• Master's Degree Social Work
• Current, valid, and unencumbered Tennessee Licensed Master Social Worker or Licensed Clinical Social Worker
• Minimum two years of professional experience as a MSW or LCSW
• 1 year of recent Home Health experience preferred
• Current and valid driver license, transportation, and auto insurance
• Working knowledge of Medicare and OASIS assessments
• Geriatric Care Management experience preferred
