Overview

We are seeking a compassionate and experienced After Hours Hospice Registered Nurse (RN) to join our hospice care team serving Butler, Pennsylvania, and nearby communities. The ideal candidate will be an empathetic and organized RN who is skilled in providing exceptional hospice care and managing after-hours patient needs.

Key Responsibilities

Provide management and delivery of individualized hospice patient care plans

Conduct in-home visits, assess patient needs, and implement appropriate nursing interventions

Support patients and families throughout the end-of-life care process

Collaborate with the interdisciplinary hospice care team to ensure continuity and quality of care

Maintain timely and accurate documentation in the EMR system

Manage after-hours calls, prioritize urgent visits, and complete charting deadlines

Utilize tablets, mobile devices, and EMR software for care coordination and reporting

Schedule and Shift Details

Schedule : Monday through Thursday, 5 : 00 p.m. 8 : 00 a.m.

Full-time, after-hours position

Travel

Local travel required within the Butler and surrounding service areas

Qualifications

Required

Associate degree in Nursing or higher

Current Registered Nurse (RN) license in the state of Pennsylvania

Minimum of 2 years of hospice nursing experience

At least 1 year of experience using electronic medical record (EMR) systems

Valid Driver's License, reliable transportation, and active auto insurance

Preferred

Experience with TruChart EHR or other EMR platforms

Strong background in home health, hospice, or palliative care nursing
