We are looking for a compassionate and skilled Nurse Practitioner to provide comprehensive primary care services to residents of assisted living facilities. The ideal candidate will have experience with the geriatric population and / or long-term care, and a commitment to delivering high-quality, patient-centered care.

Essential Duties and Responsibilities

• Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
• Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
• Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
• Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
• Participate in care initiation for our chronic care management services.
