Licensed Social Worker

A company is looking for a Social Worker to provide resource coordination services and support to program participants with medical, behavioral, and social needs.

Key Responsibilities

Conduct needs assessments and collaborate with care team members to develop individualized care plans

Identify and address barriers to care and support program participants in navigating healthcare systems and community resources

Document assessments and interventions while monitoring participant progress to achieve desired health outcomes

Required Qualifications

Bachelor's Degree in Social Work (BSW) from an accredited institution required

Master's Degree in Social Work preferred

2+ years of experience in a healthcare, hospital, or managed care setting preferred

Experience in developing and implementing new roles or programs from the ground up

Must maintain an active, current, and unrestricted license in Social Work
