Social Worker (LMSW / LCSW) – Direct Hire | White Plains, NY

Facility : White Plains Hospital – Montefiore Health System

Department : Social Work

Schedule : Monday–Friday, 8a–4p or 9a–5p; 1 weekend / month; alternating holidays

Status : Full-Time, Direct Hire | Start Date : 11 / 6 / 2025 | Term : 36 months

Hours : 40 per week

Compensation & Benefits :

• Salary Range : $87,658 – $131,487
• $5,000 Sign-On Bonus

Job Summary :

Provide comprehensive social work services to patients and families, coordinating care plans, facilitating transitions, and ensuring access to needed post-discharge resources. Serve as a liaison among patients, families, and the healthcare team to improve outcomes and satisfaction.

Key Responsibilities :

Conduct comprehensive psychosocial assessments and develop individualized care plans.

Coordinate intra- and inter-hospital care and transitional planning.

Provide patient and caregiver education and arrange aftercare services.

Collaborate with commercial payers and address high-utilizer cases.

Support goals for reduced LOS, readmission prevention, and improved satisfaction.

Document all activities per departmental standards.

Qualifications :

Master's Degree in Social Work (MSW) from an accredited program

Licensed LMSW or LCSW in New York State

Preferred : 2 years of experience in acute care or related setting

NYS SCREEN Certification (within 3 months of hire)

Flu vaccine required – no exemptions
