Summary

We are hiring for a Afterhours RN (on-call).

This is a Full Time Salaried position. On-call schedule is Monday through Thursday 4 : 30PM to 8AM (overnight). Position will include scheduled evening visits / admits if warranted. Hospice experience required.

Responsibilities

The RN Case Manager (Hospice) assumes full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies / procedures / goals / objectives of hospice to assure quality patient care.
