Healthcare Program Director

The Program Director reporting to the Executive Director oversees critical PACE (Program of All-inclusive Care for the Elderly) functions that include but are not limited to nutrition social work day center personal care services scheduling and participant addition the Program Director is accountable for developing and maintaining a high-functioning Interdisciplinary Team (IDT).

Essential Job Duties :

• Manage critical PACE functions including but not limited to hiring training supervising and mentoring staff and ensuring clear and effective workflows
• Develop plan and facilitate an effective IDT and other participant-care related meetings ensuring that they are organized and engaging and that follow-up items are documented and tracked
• Effectively oversee staff within their reporting structure to drive quality care and ensure adherence to Standard Operating Procedures (SOPs) and key regulatory requirements
• Collaborate to achieve high participant outcomes with shared participant services including but not limited to home care home health transportation central call center support and utilization management
• Oversee the WelbeHealth program site supplies and equipment management which includes monitoring and coordinating all ongoing maintenance and vendor oversight

Salary / Wage base range for this role is $145399 - $191926 / year Bonus Equity. WelbeHealth offers competitive total rewards package that includes 401k match healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
