Job Description & Requirements Specialty : Case Management Discipline : RN Duration : 11 weeks 40 hours per week Shift : 8 hours, days Employment Type : Travel Job Title : Medical Social Worker (MSW / LCSW) - Case Management / Discharge Planning Location : Laguna, California (within 50-mile radius - locals accepted at lower rate) Shift : 5x8-Hour Days (08 : 00 AM - 4 : 30 PM) Schedule : Sunday-Thursday (subject to change based on staffing) Duration : 13 Weeks Orientation : 40 hours Facility Type : Acute Care Hospital Requirements License : Active California LCSW or MSW (pending accepted, must be active by start date). Experience : Minimum 1 full year of acute care hospital experience (required). Travel experience required (no first-time travelers). Experience in discharge planning and care coordination.
