Arizona Licensed Case Manager, LVN

A company is looking for a Case Manager, LVN or LPN.

Key Responsibilities

Inform enrollees of available covered dental benefits and assist in securing dental services

Manage inquiries and conflicts related to dental care, ensuring proper documentation in the Health Solutions Plus database

Participate in multidisciplinary care team meetings and provide education on care coordination programs

Required Qualifications

Current valid license as an LVN or LPN in Arizona

4 years of experience as an LVN or LPN in public health, acute care, or case management, preferably in a dental environment

2 years of previous experience in a Case Management position

Experience in care coordination is preferred

Ability to work collaboratively in remote locations
