About the Role :

Join Skilled Nursing Pharmacy as a Staff Pharmacist and play a vital role in delivering exceptional pharmaceutical care to our patients. In this exciting position, you will collaborate with healthcare professionals to ensure optimal medication management and patient outcomes.

Responsibilities :

Review and verify medication orders for accuracy and appropriateness.

Provide clinical consultations to healthcare providers and patients.

Monitor patient medication therapy for effectiveness and safety.

Prepare and dispense medications in accordance with legal and regulatory standards.

Conduct medication therapy management and patient education sessions.

Maintain accurate records of prescriptions and patient interactions.

Collaborate with interdisciplinary teams to optimize patient care.

Participate in quality improvement initiatives and pharmacy audits.

Requirements :

PharmD degree from an accredited pharmacy school.

Active pharmacist license in California.

Minimum of 1-2 years of pharmacy practice experience preferred.

Strong clinical knowledge and ability to interpret medication therapy.

Excellent communication and interpersonal skills.

Detail-oriented with strong organizational abilities.

Proficient in pharmacy management software and technology.
