[job_card.job_description]Provide medical leadership and oversight of the interdisciplinary clinical team in a PACE (Program of All-Inclusive Care for the Elderly) setting Conduct initial and periodic assessments of participants, develop and manage medical treatment plans Participate in intake evaluations and eligibility determinations Lead quality assurance and performance improvement (QAPI) initiatives Supervise and collaborate with primary care and advanced practice providers Oversee medical policies, procedures, and protocols to ensure quality care Educate participants and their caregivers on chronic conditions and care plans Participate in on-call provider rotation as required Foster a team-based, participant-centered care environment Clear and active medical license in the State of Florida Board Certification in Internal Medicine or Family Medicine (required) Board Certification or added qualifications in Geriatric Medicine (preferred) Minimum of one year experience caring for a frail or elderly population (required) Demonstrated leadership or supervisory experience (required) CDS Certification and DEA Registration (required) Strong skills in physical assessment and chronic disease management for older adults Passion for working in an innovative, community-based healthcare setting Ability to adapt to changing priorities and patient needs
