Responsibilities
The Nurse Practitioner provides comprehensive annual risk assessments either in a residential environment or via telehealth within the scope of practice, as guided by the Collaborative Physician.
Essential Duties and Responsibilities :
Conduct thorough in-home health risk assessments to identify active and chronic disease conditions and evaluate all physical, mental, and social needs during the visit.Collect patient history, perform examinations, and establish diagnoses.Document patient visits in accordance with NCQA standards.Measure and record patient vital signs as needed.Facilitate case management referrals and collaborate with Primary Care Providers (PCP) as necessary.Communicate effectively with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure precise diagnosis and care.Perform clinical duties while adhering to OSHA Universal Precautions.Ensure patient confidentiality is maintained at all times.Attend mandatory meetings, in-services, and participate in committees as requested.Engage in professional development activities to maintain professional licenses and affiliations.You may work with :

Required Knowledge, Skills, and Experience :
Active and unrestricted Nurse Practitioner license in coverage states.Board certification through ANCC, AANP, or NCCPA.Current CPR Certification.Enrollment in Medicare / Medicaid programs.Valid driver's license with a good driving record.Exceptional EHR skills.Preferred Knowledge, Skills, and Experience :
Training or experience in geriatrics.Strong teamwork skills and the ability to maintain effective working relationships with patients, medical staff, and the public.Conditions of the Role :
Adaptability to diverse weather conditions and patients' home environments.Ability to perform a full range of motions including handling and lifting patients; require normal visual acuity and hearing; standing, bending, walking, and stair climbing are essential.Regularly lift / carry items weighing up to 50 pounds.Willingness to travel up to 150 miles daily in various settings and the ability to drive, if required.Pay Transparency
