About the Role :
The Admission Coordinator (AC) is responsible for facilitating the admission process for patients into hospice care. They will utilize their clinical expertise, compassion, and strong communication skills to assess patients for hospice eligibility, facilitate their transition into hospice services, and provide support to patients and families during a challenging time.
