About the Role

We are seeking a dedicated and compassionate  Social Worker  to join our team for a  13-week assignment  in Concord, CA. The ideal candidate will provide comprehensive social services, support patients and families, coordinate resources, and collaborate with multidisciplinary teams to ensure high-quality care.

Key Responsibilities

Conduct psychosocial assessments and develop care plans tailored to patient needs.

Provide counseling, crisis intervention, and emotional support to patients and families.

Coordinate resources and community referrals, including housing, mental health services, and financial assistance.

Advocate for patient rights and ensure continuity of care across service providers.

Document all interventions, assessments, and follow-up activities in accordance with organizational and regulatory requirements.

Participate in interdisciplinary team meetings to support patient care planning.

Ensure compliance with all legal, ethical, and organizational standards.

Job Requirements

Qualifications

Master’s degree in Social Work (MSW) required.

Active California LCSW or ASW license.

Previous clinical or healthcare social work experience preferred.

Strong communication, assessment, and problem-solving skills.

Ability to work independently and in a team-based environment.

Experience with crisis intervention, community resources, and discharge planning is a plus.
