LPN Case Manager

A company is looking for a Case Manager - LPN / LVN.

Key Responsibilities

Manage cases on a short- and long-term basis over the phone

Administer assessments to determine patients' healthcare needs and collaborate with RN Case Manager on treatment plans

Educate patients on treatment options and coordinate referrals while monitoring the effectiveness of treatment plans

Required Qualifications

Graduation from an accredited LVN / LPN program and a current LVN / LPN license

Prior experience in case management or coordinating wellness programs

Experience in medical, surgical, and / or acute care settings

Strong understanding of case management and health program coordination

Computer proficiency and working knowledge of Microsoft Office Suite
