What You’ll Do

• Provide compassionate, hands-on care in clients’ homes
• Support daily activities (bathing, dressing, mobility, meals)
• Monitor and document vital signs and changes in condition
• Administer medications and follow care plans
• Communicate with Case Managers and care teams
• Help create a safe, comfortable home environment
