Blize Healthcare is seeking a compassionate and driven Hospice Community Liaison to join our growing team. This role focuses on building and nurturing relationships with referral sources - including physicians, hospitals, skilled nursing facilities, and community organizations.
Location : Contra Costa and Alameda Counties.
Key Responsibilities :
- Develop and maintain strong relationships with healthcare providers and community partners.
- Identify and secure new referral sources through education and outreach about hospice services.
- Represent Blize Healthcare at trade shows, community events, and professional gatherings.
- Educate the community about the benefits and philosophy of hospice care.
- Monitor, track, and analyze referral and admissions data to identify trends and opportunities for growth.
- Collaborate closely with the clinical team to ensure a seamless transition to hospice care for new patients.
