Blize Healthcare is seeking a compassionate and driven Hospice Community Liaison to join our growing team. This role focuses on building and nurturing relationships with referral sources - including physicians, hospitals, skilled nursing facilities, and community organizations.

Location : Contra Costa and Alameda Counties.

Key Responsibilities :

  • Develop and maintain strong relationships with healthcare providers and community partners.
  • Identify and secure new referral sources through education and outreach about hospice services.
  • Represent Blize Healthcare at trade shows, community events, and professional gatherings.
  • Educate the community about the benefits and philosophy of hospice care.
  • Monitor, track, and analyze referral and admissions data to identify trends and opportunities for growth.
  • Collaborate closely with the clinical team to ensure a seamless transition to hospice care for new patients.

Skills and Qualifications :

  • Strong communication, interpersonal, and presentation skills.
  • Proven ability to develop and maintain professional relationships.
  • Sales or marketing experience, preferably in the healthcare industry.
  • Knowledge of the hospice care continuum, services, and regulatory requirements.
  • Associate’s degree or equivalent experience.
  • Physical capability to lift up to 50 pounds, as needed.
  • Reliable transportation for travel within assigned territories.
