A company is looking for a Quality Care Coordinator to develop and implement quality improvement strategies.
Key Responsibilities
Conduct outreach to members and providers to close quality gaps and achieve population health outcomes
Evaluate utilization patterns and medical records to monitor quality and efficiency results
Coordinate and document quality interventions and care management programs to promote effective service delivery
Required Qualifications, Training, and Education
Bachelor's degree in a related field required; Bachelor's degree in Nursing preferred
4 years of clinical RN experience preferred
Experience in data analysis, quality improvement, or HEDIS medical record review preferred
WI Registered Nurse (RN) licensure preferred
