[job_card.job_description]Santa Monica, CA 90402 Pay Shift _(Full-Time)_ Monday-Friday : 8am-5pm No weekends. Qualifications 5 years of LVN experience. Strong Phlebotomy experience. Primary Care experience. Must be able to commute to office. Graduate of an LVN program. BLS certification. The ideal candidate will exhibit professionalism, strong interpersonal skills, and excellent verbal and written communication, ensuring they can confidently engage with high-profile clients. Elation EMR system experience. Tech savvy and knowledgeable with Apple products. Application Guidelines Professional Stability : 2+ years in recent roles preferred. Job Summary Support one Physician in a small practice, including administrative tasks and direct patient care. Correspond with patients via email and provide personalized, world-class care. Assist with house calls (e.g., testing for RSV, COVID, and Flu; blood draws; IV administration; and well visit checks). Conduct routine and comprehensive health assessments for adults and children, including blood draws, swabs, lab processing, EKGs, vaccinations, and occasional support during medical procedures. Document all therapies, medications, and clinically relevant interactions in the electronic medical record. Ensure all patient labs are complete, coordinating closely with front office staff and communicating results to the Physician and patients. Participate in daily team huddles with Physicians to discuss patient care. Manage a dedicated patient panel of fewer than 10 patients per day (spending 1-2+ hours per patient), with occasional cross-coverage for adult and pediatric panels when needed. Correspond directly with patients as needed, fostering trust and reliability. Prepare for and conduct home visits independently or with an RN or Physician. Cover other paneled nurses within the LVN scope during PTO or leave. Maintain a working knowledge of applicable Standards of Practice.
