Med Surg Rehabilitation - Responsibilities
- Completed FIM Training and assesses / document patients' functional levels and assistance needs.
- Provides patient / family education on rehab equipment and safety precautions for post-surgical and CVA patients.
- Performs comprehensive admission assessments, including vitals, history, psychosocial needs, and patient education needs.
- Develops and initiates interdisciplinary plans of care within required timeframes.
- Prioritizes and organizes patient care based on clinical needs and shift requirements.
- Performs accurate system assessments each shift and responds appropriately to diagnostic data.
- Demonstrates understanding of patient diagnoses and implements nursing interventions safely, including during emergencies.
- Administers medications and IV therapy per policy, documenting effectiveness and reporting adverse reactions promptly.
- Provides individualized patient / family instructions and maintains clear communication regarding the plan of care.
- Ensures daily hygiene, performs pre- / post-operative care, and adheres to all infection control and safety precautions.
- Performs wound care, dressing changes, ostomy care, catheter insertion, glucose monitoring, specimen collection, and tube feedings.
- Reports abnormal findings immediately and initiates wound / skin protocols based on risk.
- Transfers patients safely, completes transfer summaries, and ensures proper documentation and communication during hand-offs.
- Demonstrates correct use of equipment and restraints while ensuring safe care delivery.
- Evaluates, documents, and updates patient progress, outcomes, care plans, and discharge needs each shift.
- Maintains professional development through mandatory in-services, competency assessments, staff meetings, and effective communication.
- Provides leadership through delegation, participation in PI activities, collecting PI data, and documenting / charging supplies daily.
