Comments And Notes: 06/01 Start date only - BILH coversheet required at time of submission. 10H days, Mon, Tues, Thurs, Fri 7:30a-6p. Pain clinic experience required - no call and no holidays. Must have the following pain procedure experience: Lumbar Transforaminal Epidural Steroid Injection (TFESI), Lumbar Interlaminar Epidural Steroid Injection, Cervical Interlaminar Epidural Steroid Injection, Cervical Transforaminal Epidural Steroid Injection, Thoracic Epidural Steroid Injection, Lumbar Facet Joint Injection, Cervical Facet Joint Injection, Medial Branch Block (Cervical, Thoracic, Lumbar), Sacroiliac (SI) Joint Injection, Greater Occipital Nerve Block, Third Occipital Nerve Block, Intercostal Nerve Block, Cluneal Nerve Block (Superior, Middle, Inferior), Pudendal Nerve Block, Genicular Nerve Block, Suprascapular Nerve Block, Piriformis Muscle Injection, Ilioinguinal/Iliohypogastric Nerve Block, Lateral Femoral Cutaneous Nerve Block, Trigger Point Injection, Radio Frequency ablation, Caudal, Blood patch. NO local travelers will be accepted (cannot reside within 50 mi from the facility) Start date: ASAP starts (looking for end of March, beg of April) Orientation is weekly (every Monday barring a holiday) NM to interview and offer Please provide dates and times available for interview at time of submission ALL RTO REQUESTS MUST BE PRESENTED AT TIME OF SUB Travelers who have worked for *** as travel, perm or per diem within the last 6 months will not be accepted - MUST be separated from facility for 6 months to be considered.
