Job Description

Job Title : Registered Nurse I

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : None

Experience : Minimum 1 full year of Adult Home Health Experience or Hospice experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase (HCHB)
Proficiency in OASIS assessments
Reliable transportation, valid driver's license, and liability insurance

Job Description :

Provides, coordinates, and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures.

Supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Responsible to the Clinical Manager.

Perform home visits with an average of 5-8 patients per day.

Attend periodic office meetings.

Assignments may require traveling up to 75 miles from the home clinic.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Documentation must be completed within 24 hours of the patient appointment.

Maintain weekly productivity of 30 points, equating to a 40-hour workweek.

Must be flexible with on-call rotations as required by location.

Local candidates accepted at the same rate.

Must have experience using HomeCare HomeBase EMR within the last 6 months or complete a retraining course during compliance.
