Position Summary :

The Home Health Quality & Compliance RN is responsible for ensuring regulatory compliance, maintaining high-quality patient care standards, and supporting performance improvement initiatives within the home health program. This role conducts utilization reviews, performs chart audits, oversees OASIS integrity and submissions, and promotes adherence to state and federal home health regulations.

Essential Duties and Responsibilities :

Implement, monitor, and evaluate performance improvement, quality assurance, and utilization review activities.

Conduct thorough reviews of medical records—including plans of care, clinical documentation, and progress notes—to ensure accuracy, completeness, and compliance with regulatory standards.

Facilitate interdisciplinary case conferences and ensure ongoing coordination of care.

Monitor OASIS data integrity, ensure timely correction of discrepancies, and complete all OASIS data submissions.

Oversee the development, implementation, and updates of individualized plans of care in accordance with patient needs and regulatory requirements.

Generate monthly reports to assess service quality and monitor publicly reportable metrics.

Assist in evaluating clinical staff competence and compliance with established standards.

Maintain a strong knowledge of current home health regulations, rules, and clinical best practices.

Ensure consistent compliance with federal, state, and accreditation standards in home health operations.

Promote a clean, safe, and compliant patient care environment using SAFE error-prevention processes.

Protect patient and employee confidentiality at all times and uphold all Standards of Conduct.

Required Qualifications :

Minimum 2+ years of Home Health experience – REQUIRED

Previous Quality or Performance Improvement experience – REQUIRED

COS-C (Certificate for OASIS Specialist–Clinical) – REQUIRED

OASIS documentation and review experience – REQUIRED

Valid driver’s license and current auto insurance – REQUIRED

Copy of agency’s Illinois nursing license must be included with submission – REQUIRED

Current and active Registered Nurse (RN) license

Strong understanding of home health regulatory compliance and documentation standards

Excellent analytical, communication, and organizational skills
