Part time Weekend Social Worker

The Social Worker Home Health and Hospice is a member of the Interdisciplinary Health team and is responsible for providing social work services in the area of Hospice care. The social worker may provide direct care, teach, supervise and consult. The social worker provides service under written orders of the patient's physician and the Team's Plan of Care and under continuing medical direction.

JOB DUTIES AND RESPONSIBILITIES :

• Visits hospice patients and families to complete psychosocial assessment provide education and support.
• Assists patients and families in resolving personal and environmental difficulties, by providing information, referral, coordination and advocacy as needed to enhance their ability to cope with factors related to their illness.
• Counsels and assists patients and their families to participate in the development of the hospice plan of care.
• Serves as supportive caregiver to the Hospice Interdisciplinary Team.
• Responsible for the development, implementation and timely revision of the care plan in coordination with the family and hospice team.
• Participates in interdisciplinary team meetings providing Psychosocial assessment information relevant to the plan of care.
• Assesses, documents and reports to the hospice interdisciplinary team the social and emotional factors concerning patient’s capacity and potential to cope with problems of daily living.
• Accurately completes all documents pertaining to patient care and services provided within 24 hours in a neat, timely, accurate, concise and legible records and reports. Other duties as required, including coordination and assignment of social service referrals, participation in the agency’s Ethics Committee and Crisis Intervention Team as appropriate.
• Is available for emergency on-call service.
• Uses own transportation to provide services : Must have and maintain valid PA or NJ Driver’s License; Must have and maintain a good driving record accepted by the Network insurance carrier; Must have and maintain motor vehicle insurance with the minimum requirements established by PA or NJ; Must obey and adhere to all motor vehicle regulations and laws while operating vehicle; Must perform routine maintenance on vehicle.

PHYSICAL AND SENSORY REQUIREMENTS :

Medium Work :

Exertional activity

Occasionally lift and / or carry 50 lbs. Frequently lift and / or carry 25 lbs. Stand and / or walk at least 6 hours in an 8-hour work day.

Nonexertional activity

Frequent stooping (bending at the waist) and crouching (stooping and bending at the knee). Frequent Handling (grasping). Occasional Fingering (fine manipulation).

Important : The functional capacity to perform medium work includes the functional capacity to perform sedentary and light work.

EDUCATION :

Master’s Degree required. Current, acceptable professional license to practice Social Work in the Commonwealth of Pennsylvania preferred.

TRAINING AND EXPERIENCE :

Two (2) years of social work experience in health care setting prefer home health, oncology or hospice experience preferred.
