Arizona Licensed RN Case Manager

A company is looking for an RN Case Manager to manage care for members with complex medical and behavioral needs.

Key Responsibilities

Engage members to complete comprehensive needs assessments and develop person-centered care plans

Collaborate with internal care teams and community resources to implement care plans and support member self-management

Manage care transitions and advocate for members and families to ensure their needs are represented

Required Qualifications

Current, unrestricted independent licensure as a Registered Nurse in Arizona

4+ years of clinical experience

1+ years of experience with MS Office, including Word, Excel, and Outlook

Must reside in Arizona
