We are looking for a motivated Nurse Practitioner to join our medical group full-time in Melbourne and Maitland FL. We are a highly reputable, growing multidisciplinary team that believes in providing high quality care and helping others improve their quality of life. We are seeking a like-minded nurse practitioner to join our team! In this role, you will have the opportunity to provide care and support to a wide variety of patients and work in a friendly, welcoming environment. The ideal candidate is personable, enjoys focusing on getting the best outcome for the patients, and is a team player willing to work with a multidisciplinary team and is bilingual (Spanish / English). This position is in Maitland (3-4 days per week) with potential for additional pay / hours if willing to commute 1 day per week to our office in Melbourne.

Duties :

• Combination of telehealth and in office patient visits
• Patient exam / evaluation- HPI, physical exam, medical history, review pertinent information / records / imaging
• Order and review diagnostic imaging / labs, and testing
• Diagnosis and Treatment Plan formulation
• Referral to other providers as indicated
• Documentation- timely and accurate
• Patient education- as to the diagnosis, treatment plan options and pre / post care instructions
• Collaboration with the multidisciplinary team for comprehensive patient care
