[job_card.job_description]Senior Care Long-Term Care (LTC) Board Certified in Family Medicine or Internal Medicine Active Licenses / Certifications (All Required) : DEA & CDS Registration in CA AHA Basic Life Support (BLS) Certification Valid California Drivers License , reliable personal transportation, clean driving record, and auto insurance Preferred : PACE experience (1+ year) Bilingual in Spanish Deliver holistic primary care to PACE participants, including exams and chronic disease management Manage your own patient panel in coordination with a complex care RN Initiate and review orders for medications, diagnostics, and referrals Provide care across PACE center, participant homes, SNFs, ALFs, and hospitals Collaborate with Interdisciplinary Team (IDT) including behavioral health, pharmacy, and rehab Participate in care planning, Service Determination Reviews , and clinical team meetings Document accurately in the EMR Participate in after-hours phone call rotations , supported by nurse triage Monday-Friday, 8 AM 5 PM Flexible daily schedule with occasional travel to care settings Rotating on-call (remote) coverage 401(k) with Employer Match Six medical plans up to 80% coverage for you / 75% for dependents Dental, Vision, FSA, HSA, Short / Long-Term Disability PTO : 20 days / year + 12 paid holidays + 2 floating holidays CME / CEU budget + paid time off for professional development Home office stipend for hybrid / remote setup Family-focused policies, including paid parental leave
