Delivering personalized high-quality care to seniors in their homes, assisted living, hospice and nursing facilities.Delivering dementia-informed, relationship-based care across the continuum of care.Conducting comprehensive assessments and developing tailored care plans for older adults.Managing chronic conditions and performing care with skill and empathy for the patients and their families.Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.Maintaining consistency in documentation, follow-through, and patient communication.Compensation and Benefits
