The nurse will deliver compassionate, multidisciplinary, coordinated care for all breast patients through comprehensive and personalized patient-centered care. Work in partnership with breast cancer patients and their families through prevention, screening, diagnosis, treatment, and survivorship. Provide patient education and coordinate care for patients who have been diagnosed with breast cancer or other breast conditions. Collaborate within a multidisciplinary team that includes genetic counselors, breast surgeons, plastic surgeons, medical oncologists, radiologists, radiation oncologists, and advanced practice providers. Utilize targeted assessment skills to assess patients during visits and via telephone triage. Support and facilitate the coordination of care for patients, including setting up appointments for procedures, imaging, labs, and infusion therapy. Provide education to patients regarding treatment options and help guide them through their treatment process. Assist in answering telephone triage calls, addressing symptom management concerns, and coordinating care for patients as needed.
