Speech-Language Pathologist (SLP)

Location : Hayward, CA

Contract Length : 14 Weeks

Job Description

We are urgently seeking a skilled and compassionate Speech-Language Pathologist (SLP) to support a post-acute rehabilitation facility. The ideal clinician will have strong assessment and treatment skills, work well within an interdisciplinary team, and provide patient-centered care to adults and geriatric patients.

Responsibilities

  • Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations.
  • Develop individualized treatment plans based on patient needs and clinical findings.
  • Deliver evidence-based therapy focused on communication, cognition, and dysphagia management.
  • Document evaluations, daily notes, progress reports, and discharge summaries in accordance with facility and professional standards.
  • Collaborate closely with PT / OT, nursing, physicians, and interdisciplinary team members to ensure optimal patient outcomes.
  • Educate patients, families, and caregivers on therapy techniques, communication strategies, and safe swallowing practices.
  • Participate in care conferences and interdisciplinary meetings as needed.
  • Maintain compliance with ethical standards, professional guidelines, and state licensure requirements.

Requirements

  • Active SLP license (state-appropriate).
  • CCC-SLP strongly preferred; CFs not accepted unless specified.
  • Experience in skilled nursing, post-acute, or geriatric rehab preferred.
  • Strong clinical documentation and communication skills.
  • Ability to adapt to fast-paced environments and diverse caseloads.
