Speech-Language Pathologist (SLP)

Job Description
We are urgently seeking a skilled and compassionate Speech-Language Pathologist (SLP) to support a post-acute rehabilitation facility. The ideal clinician will have strong assessment and treatment skills, work well within an interdisciplinary team, and provide patient-centered care to adults and geriatric patients.
Responsibilities
- Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations.
- Develop individualized treatment plans based on patient needs and clinical findings.
- Deliver evidence-based therapy focused on communication, cognition, and dysphagia management.
- Document evaluations, daily notes, progress reports, and discharge summaries in accordance with facility and professional standards.
- Collaborate closely with PT / OT, nursing, physicians, and interdisciplinary team members to ensure optimal patient outcomes.
- Educate patients, families, and caregivers on therapy techniques, communication strategies, and safe swallowing practices.
- Participate in care conferences and interdisciplinary meetings as needed.
- Maintain compliance with ethical standards, professional guidelines, and state licensure requirements.
Requirements
Active SLP license (state-appropriate).CCC-SLP strongly preferred; CFs not accepted unless specified.Experience in skilled nursing, post-acute, or geriatric rehab preferred.Strong clinical documentation and communication skills.Ability to adapt to fast-paced environments and diverse caseloads.Additional Information
