Description :
The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care based on nursing theory and research to a designated group of patients and significant others.
Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care through the nursing process based on theory and evidence-based practice.
Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
Integrates relevant assessment and intervention skills in the delivery of nursing care.
Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care while delegating to others as appropriate.
Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention.
Communicates effectively and professionally with patients, families, and all members of the interdisciplinary care team.
Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates effective allocation of material resources.
Role models the tenets of Relationship Centered Care through facilitation of vital actions such as admission welcome, bedside shift report, and purposeful rounding.
Considers patients' values, preferences, cultural diversity, and knowledge in all aspects of care.
Establishes and maintains a therapeutic relationship with patients and families.
Uses evidence-based practices to enhance understanding of patients' perceptions of care.
Initiates service recovery for patients and families with concerns and escalates issues as needed.
Demonstrates caring, respect, compassion, empathy, and active listening through dialogue and body language.
Supports staff in utilizing evidence-based practices to improve understanding of patients' perceptions of care.
Practices safe and efficient patient-centered care, identifying and addressing barriers and opportunities for improvement.
Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served.
Implements process improvement strategies based on institutional evidence and procedures.
Engages in peer and institutional review processes.
Collaborates with interdisciplinary teams to create and implement quality improvement projects and nursing research studies.
Contributes to quality and safety practices and required compliance measures.
Implements high reliability principles to identify and avoid high-risk behaviors to ensure patient safety.
Escalates potential safety hazards or gaps from best practice following appropriate protocols.
Documents nursing practice accurately and thoroughly to support quality and performance improvement initiatives.
Performs charge nurse duties as assigned.
Coordinates with management to ensure appropriate utilization of supplies and inventory for patient care.
Escalates material or staffing deficits as necessary.
Demonstrates patient-focused and cost-effective approaches to care involving equipment, staff, and supplies.
Facilitates patient throughput on a shift basis.
Contributes to a supportive environment for clinical colleagues, patients, and their families.
Advances clinical competence in nursing practice moving from novice to expert.
Projects a professional image and communicates with respect and compassion.
Incorporates ethical principles into decision-making for patients and families.
