Status : Salaried, Full-time, Exempt

Position Objective :

The Outreach Services Coordinator position is accountable to the client, Horizon House, and the Professional Blended Street Outreach (PBSO) Team. Accountability occurs within the context of ethical standards, organizational policies and procedures related to the delivery of service, PBSO standards of practice, and the written agreement between Horizon House and relevant funder(s).

This Outreach Services Coordinator position is 100% funded through by a grant from the Nina Mason Pulliam Charitable Trust (The Trust). As such, this position is dependent on execution and continuation of said grant agreement, and termination or nonrenewal of the agreement will result in termination of the Outreach Services Coordinator position.

Requirements :

Training / Education :

• Bachelors degree preferred; commensurate experience will be considered
• Case management experience (3-5 years)
• Homeless services experience strongly preferred
• Bi-lingual (English / Spanish) a plus

The Street Outreach Services Coordinator will regularly communicate with Horizon House employees, outside vendors and other individuals as deemed necessary to fulfill the responsibilities of the position.
