[job_card.job_description]Deliver evidence-based therapeutic interventions to support clients' emotional and behavioural needs. Provide individual and family counselling to achieve treatment goals. Case Management : Facilitate coordination with community resources, schools, and healthcare providers. Maintain accurate and timely documentation in compliance with Medicaid and agency standards. Crisis Intervention : Coordinate with the Team Lead for support on high-risk situations. Participate in on-call rotations to provide 24 / 7 support for clients in crisis. Collaboration and Advocacy : Advocate for clients needs within the healthcare and educational systems. Attend supervision sessions and team meetings to review cases and enhance services. Education : Bachelors or masters degree in a human services field (e.g., social work, psychology, counselling) with eligibility as a Qualified Professional under North Carolina state guidelines. Experience : Minimum of two years working with children and families in a behavioural health setting. Skills : Proficiency in crisis management and documentation. Job Description : Qualified Professional (QP) - Intensive In-Home Services
Position Summary :
The Qualified Professional (QP) is an integral member of Arbor Care Solutions Intensive In-Home (IIH) team, delivering therapeutic services to children, adolescents, and families in crisis. Under the supervision of the Team Lead, the QP develops and implements treatment plans, coordinates care, and supports clients in achieving stability and resilience.
Key Responsibilities :
- Direct Client Support :
Conduct initial assessments and collaborate on developing person-centred treatment plans.
Monitor client progress, document services provided, and adjust treatment plans as needed.
Respond to client crises, implementing de-escalation techniques and safety plans.
Work closely with the AP to ensure a cohesive approach to client care.
Qualifications :
Strong clinical skills in treatment planning and therapeutic interventions.
